New Client Consent Form

I acknowledge that the hairstylists of the salon are licensed professionals and should be treated with respect.

I release the salon and the hairstylist from liability for dissatisfaction when proper procedures have been followed.

I am allowing the salon to apply necessary chemicals and products as part of the service during my hair appointment and release the salon from liability for adverse reactions.

I understand that the result of the service may vary from one person to another.

I agree that the color, cut, and style is final after the service is completed. If there are any changes to be made 1 week after the service ends, the client will be charged.

I confirm that I will follow the after-care regimen and the suggested follow-ups in maintaining and treating my hair.

I consent to the salon to taking photographs of the provided service and sharing photographs to social media for marketing.
